Requirements Prior to Your Final Appointment:
To ensure your clinician can complete a thorough and accurate evaluation, you may have been asked to complete online rating forms or submit records. Please review the “Testing Confirmation” or “Testing Appointments” messages previously sent to your patient portal.
If you have already opened these messages, you may need to expand the “Read Messages” bar located at the bottom of the screen to access them.
Please complete any outstanding forms and submit all required documentation as soon as possible to avoid delays with your final appointment or review note.
-
During Your Appointment:
During your final appointment, your clinician will review the evaluation findings and recommendations with you.
If you have any upcoming appointments or obligations that require written documentation, please inform your clinician at that time.
-
After Your Appointment:
Following your appointment, your clinician will complete a summary review note outlining the evaluation results, diagnosis, and recommendations.
Please note that we are unable to provide a specific timeline for review note completion, as additional time may be required for review and supervisory approval to ensure accuracy and quality.
In order to avoid delays in the completion of this documentation, ensure any outstanding balance on your account has been paid.
Once your review note is complete, it will be uploaded to your patient portal and you will receive an email notification when it is available.
-
Accessing and Sharing Your Review Note:
Your review note will be accessible through the patient portal. You may download and/or print this document to share with other providers or agencies (e.g., physicians, schools, etc.).
If you were referred by a physician and we have the appropriate authorization on file, we will also forward the review note directly to that provider.
If you are applying for programs or services (e.g., SSI/SSD, OPWDD, SPOA, IEP), it is your responsibility to submit the review note with your application. Failure to do so may delay access to services.
-
Ongoing Services with AAPSA:
If you indicate interest in any recommended services with us at the time of your review appointment, your case will be placed on the appropriate waiting list. Please keep us informed of any changes to your insurance, contact information, availability, or preferred office location.
Due to limited availability, it is important that you promptly return phone calls or portal messages and notify us of any updates so we can schedule services as soon as possible.
