Patients Aged 1-8

Initial Parent Appointment: Unless requested or advised otherwise at the time of scheduling, your initial appointment will be held via telehealth. The patient’s parent or guardian should attend this appointment. The patient should not be present. Please be aware that if your appointment is via telehealth, a distraction free and private area is necessary for the session to be held. Sessions will be held via Zoom, so ensuring a stable internet connection is important.

Initial Patient Appointment for Patients Aged 1-8: This appointment will be an observation appointment of the patient with an adult in a natural environment, where you and the child have the opportunity to interact as you might at home. Please note that results and recommendations will not be provided during this appointment. Follow-up testing appointments will be scheduled subsequently, pending insurance authorization if applicable.

Patients Aged 9-17

Initial Patient Appointment for Patients Aged 9-17: This appointment will be an initial youth appointment between the clinician and patient alone. Please note that results and recommendations will not be provided during this appointment. Follow-up testing appointments will be scheduled subsequently, pending insurance authorization if applicable.

Patients Aged 18+

Patients 18 Years & Older – Your initial appointment will be held in office unless otherwise discussed at the time of scheduling. You must personally attend this appointment, but may bring a parent/caregiver to the appointment to assist you in providing historical information if you choose. Please note that results and recommendations will not be provided during this appointment. Follow-up testing appointments will be scheduled subsequently, pending insurance authorization if applicable.

*If you have any appointments via Telehealth/Zoom, we recommend that a desktop computer, laptop, or tablet be used and a wired internet connection to ensure a stable signal. Zoom will need to be installed on your device or computer prior to your first session. Due to HIPAA and safety concerns, telehealth sessions cannot be held while driving, working, or in a public setting. If the environment is not conducive, your clinician may discontinue the session and you will be asked to reschedule. Please sign into your meeting at the scheduled time. Late attendance may result in the session needing to be rescheduled. The links for meetings can be found on your Patient Portal, but will also be sent via email at the time your clinician begins the session. If there are concerns with your ability to meet these expectations, please let us know so we can make arrangements for the session to be scheduled in-office.

Billing Information
It is our policy that payment is required at the time of service. We kindly ask that you enter your credit or debit card information on your online Patient Portal before scheduling. Our office requires a credit or debit card to be kept on file for the processing of patient cost share for virtual appointments. However, for in-office appointments or prepayment of virtual sessions, we accept payment in the form of cash, check, or credit/debit card.

Please follow the steps below to input your credit card information.

3. Select “Add New Card”
4. Set up Signature
5. Input credit card information
6. Select “Submit”

Appointment Cancellation Or Insurance Changes
Please note that any cancellation of appointments may cause a delay in the completion of your evaluation. If your insurance provider has changed or if you need to cancel/reschedule any of these appointments, please give our office as much notice as possible; at least 48 hours notice is required to avoid a cancellation fee.

Please note that our administrative team is currently experiencing a high volume of calls and new patient requests, which may lead to extended response times when we receive your completed forms. We are currently experiencing a delay in scheduling and are unable to provide an exact timeline of when we will be reaching out to schedule your appointment or your position on the waitlist. Please know that we process these requests in the order they are received and are making calls as quickly as we can.

We will be reaching out by phone to schedule, so if at any point your phone number changes please let us know. To avoid any further delays in scheduling, please also ensure to notify us if you have a change in insurance.

Please submit the following records to our office at your earliest convenience. You may attach any relevant documents through the portal messages; our staff will ensure they are correctly uploaded to the client’s chart. Whenever possible, kindly submit documents in PDF or Word format, as image files may require additional processing for clarity. Thank you for your cooperation.
